XLTEK’s Accessories have been recognized for their quality design and
reliability for performance. If for any reason you feel that you have received
a product that does not meet your satisfaction, call us within 30 days of
receiving the product to discuss further. Please contact our Accessories
Department at 1-800-387-7516 ext 272.
Accessories Orders & Inquiries
All orders placed prior to 1:30pm (EST) are processed the same day of
receipt. If due to unanticipated demand,we do not have your entire order
available for immediate shipment we will ship the products available and
the remaining portion of the order will be expedited as soon as it is
available. An Accessories Representative will contact you the day your
order is received if this situation were to occur. XLTEK Accessories Sales
Representatives are available from Mon - Fri 8:30am to 5:30pm (EST) to
answer any questions regarding your order or our products.
Special Orders
Special ordered items may take 2-4 weeks for delivery. An Accessories
Representative will be able to provide you with a detailed delivery time
when requesting a special order.
Your Feedback
We are interested in what you have to say about our service, and product
selection. If there is a product that you use and notice that it is not
available in our existing product offering, let us know! We are continuously
striving toward meeting all your accessories supply needs.
Pricing and Product Specification
We reserve the right to change product specifications, availability and
prices without prior notification. While we make every effort for it
not to happen, XLTEK is not under a contractual agreement for
typographical errors.
Shipping and Delivery
XLTEK prepays and adds the shipping charge to your invoice. All orders
are shipped Regular Ground service unless you require an expedited
shipment which our Accessories Representatives will be glad to arrange
upon request. Posted shipment rates may vary due to destinations outside
of North America and weight. An Accessories Representative will be
happy to assist in providing you with the necessary information. Orders
shipped from our warehouse in the United States are FOB Cheektowaga,
NY and orders shipped from our warehouse in Canada are FOB
Oakville, Ontario.
Payment Terms
XLTEK bills on a net 30-day basis to accounts with approved credit. For
your convenience VISA or MasterCard are also accepted.
If paying by check or money order, please send to: XLTEK, 2568 Bristol
Circle, Ontario, Canada L6H 5S1. A finance charge may be applied to
overdue accounts.
Sales Tax
Catalog prices do not include any applicable taxes. XLTEK is required by
law to collect all state and local sales taxes where applicable. Please
provide us with a copy of your exemption certificate for our records.
Prices
Due to market fluctuations our prices are subject to change. All prices are
stated in U.S. dollars.
Return Information
Please contact an Accessories Sales Representative in order to obtain
approval for a Return Material Authorization (RMA#) and shipping
instructions. Items marked with an RMA# will be handled quickly and
efficiently, items without an RMA# will not be accepted. Returns may
be subject to a 20% restocking fee.
For safety and health reasons, needle or other invasive electrodes cannot
be returned if they have been removed from their original packaging.
Liability
XLTEK is not responsible for injury, infection or other damage resulting
from the use of the electrodes supplied. Electrodes are for medical
professional use only. Sterilization (where applicable), use and application
of electrodes is the responsibility of the user. Federal law in the U.S.A.
and Canada restrict the sale, distribution or use of these products to, by,
or on the order of a licensed medical practitioner.
General Electrode Care
XLTEK recommends that you inspect all products upon receipt and prior to
each use. Electrodes that show wear or damage that would impair a study
should be replaced.All disposable needle products remain sterile if the seal or
pouch is not opened or damaged and when used prior to the expiration date.
Sterilization
Please follow the manufacturers sterilization directions and guidlines
that are provided with the product. For detailed sterilization information
refer to the Association for the Advancement of Medical
Instrumentation Standards and Recommended Practices for Sterilization
(www.aami.org).
